Summary:1908 1908 1900s 18 pages 7 his contributions and the secretary's salary ; but before such member can receive any sick pay he must produce a certificate from a medical man, when, if the stewards feel satisfied, his money will be transmitted to him, but he must pay the expenses of transmission. The medical certificate to be renewed every week. Any country member declaring on the funds of the society shall be furnished with a sick list, the same to be signed by two householders not being publicans; the same to be sent to the secretary before he can receive any sick pay. Any member declaring on the sick fund shall send in writing to the secretary the nature of his complaint, or in default be fined two shillings and sixpence. FORM OF DECLARATION ON THE FUNDS. TO THE SECRETARY OF THE UNITED BROTHERS' BIRMINGHAM BENEFIT SOCIETY. Sir, This is to Certify that I, residing at being afflicted with and not able to follow my employment, and being a Free Member of the above society, do declare on the funds thereof, this day of 190 Signed DECLARATION OFF THE FUNDS Sir, This is to Certify that I, residing at having been afflicted with but finding myself able to resume my employment, declare off the funds this day of 190 Signed 345/7/2/2
