Applications are invited from all persons under 60 years of age resident and/or employed within the Metropolitan Area as prescribed by the King's Fund, and from members of Associations whose registered offices are within that area, also from employees whose company's staff, welfare or personnel department, is directed from offices within the area. There is no income limit. HOW DO I ENROL? Complete the form of application and send it, accompanied by a remittance to cover a period of not less than three months (13 weeks) subscription, or a Banker's Order as may be more convenient to THE SECRETARY, THE LONDON ASSOCIATION FOR HOSPITAL SERVICES, 10 OLD JEWRY, LONDON, E.C.2. If you desire to pay your subscription weekly, monthly or quarterly from your salary, hand the completed form to the office group receiver. Further facilities for enrolment will be provided at the voluntary hospitals. HOW TO OBTAIN BENEFIT Notify the Secretary before (if possible) or immediately treatment is arranged. A claim form will then be issued for completion, and should be returned to the Secretary within one month of leaving the hospital or nursing home. This form must be accompanied by the receipted accounts for maintenance and medical or surgical fees.
