xxii. APPENDIX. XVI.— BENEFITS. (a) MEDICAL ATTENDANCE AND MEDICINE. The Medical Officers will attend at the homes of the members, including the outer boundaries of Bromborough and Brimstage and within the borough of Birkenhead. (b) SICK BENEFITS. CLASS A.—3/- per week for the first 13 weeks, half-benefit for the second 13 weeks, and quarter-benefit until declared off the Society. CLASS B.—5/- per week for the first 13 weeks, half-benefit for the second 13 weeks, and quarter-benefit until declared off the Society. CLASS C.—10/- per week for the first 13 weeks, half-beneRt for the second 13 weeks, and quarter-benefit until declared off the Society. (c) FUNERAL BENEFIT. CLASS A.— £3 on death of Member. CLASS B.— £4 on death of Member. „ £3 on death of Member's Wife. „ £1 on death of Member's Child under 14 years, if born alive. CLASS C.— £8 on death of Member. „ £6 on death of Member's Wife. „ £2 on death of Member's Child under 14 years, if born alive. (d) MEDICAL AID FOR MEMBERS' FAMILIES. Subject to the approval (in each case) of the Committee and Medical Officer a member may obtain medical aid and medicine for his wife and family on payment of the following additional subscriptions : — For his wife ................................. 1d. per week. (This does not include maternity cases, or complications arising therefrom, but after the lapse of one month following accouchement, members' wives requiring the doctor or medicine shall be duly entitled to such benefits.) For all his Children, if under 14 years of age and residing with him......... 1d. per week. A member may enter his mother, or wife's mother, if residing with him, for medical aid only, by paying 1d. per week extra. She must be passed by the Society's Doctor before joining. MEDICAL AID AND SICK BENEFIT. XVII.—(a) No member shall be entitled to medical aid or sick benefit who has not paid 8 weekly subscriptions to the Society, except in case of accident whilst on duty, when he shall be entitled to benefit at once. NOTE. - No member shall be allowed to pay in advance so as to become entitled to immediate benefit. (b) Members receiving any hurt or accident whilst in the pursuit of, or playing at any game, sport or recreation, shall be entitled to medical aid but not to sick pay. (c) DECLARING ON THE SOCIETY.— A member in benefit who is unable to follow his employment in consequence of sickness or accident shall send a written statement within 24 hours to the Secretary, dated and signed by a duly qualified doctor. A member shall not be entitled to sick pay for any day of which he has worked any part. The Certificate to take effect the first clear day he is absent from work through sickness, and afterwards he shall be paid for each full day's sickness (Sundays excepted) at his proper rate. Sick pay to cease on date of Doctor's Declaring-off Certificate. (d) Members must obtain a Doctor's Certificate each week whilst he is on the Sick List. Should a member omit to supply a Declaring-on Note in due course, sick pay shall only commence as and from the date on which the note is handed in. (e) If a member shall have received Sick Benefit and then resume work, his previous period of sickness (in any 12 months dated from the date of his first sick payment) will be reckoned against him should he again declare on the Society. Should a member at different intervals during any 12 months as above, have received 13 weeks' sick pay, any subsequent sick pay shall be at the rate of half-benefit for the next 13 weeks and quarter-benefit until declared off the Society, and where sickness is continuous, the commencing of a new financial year shall not entitle the sick member to an increased rate for that sickness. If a member shall have received 12 months, or more, sick benefits, and declared off the Society, a period of 13 weeks, in full work and benefit, must elapse before he can again declare on the Society, after the lapse of that time he would be entitled to full benefit. In case of accident whilst on duty, however, the Committee to have power to modify this rule. (f) A member whilst receiving sick allowance must continue to pay his usual subscription. (g) SICK BENEFIT WHEN IN ARREARS.— Any member who is 2 weeks' contributions in arrears shall not be entitled to Sick Benefit until all the money due to the Society has been paid up. And if 4 weeks' in arrears, he shall not be entitled to Sick Benefit until 14 days after he has made himself clear on the books. (h) MEDICAL AID WHEN IN ARREARS.— No member whose Subscriptions are not fully paid up is entitled to medical aid. (i) DECLARING OFF THE SOCIETY.— Every member on his returning to work after sickness, must notify the Secretary thereof the same day, and produce the Doctor's Declaring-off Certificate, under penalty of one day's sick pay. (k) PAYMENT OF SICK MEMBERS.— A Sick Note bearing date not earlier than Friday of the same week, must be signed each week by a Doctor, and forwarded to the Secretary before 12-30 p.m. on Saturday, at which time, if the above regulations have been complied with, all duly certified claims will be paid by the Secretary. FORFEITURE OF SICK PAY. XVIII.— If any member receiving benefit from the society shall do any kind of work, or transact any business on his own account for profit, or shall refuse to see or give information to the Steward respecting the state of his health or nature of his employment, or the cause or nature of his infirmity, or shall be found going into or on the premises of any public house (except he resides there), or visiting any place of public amusement, or be absent from his usual place of abode later in the evening than six o'clock during the winter (from October 1st to the last day of March), or eight o'clock in the summer (between April 1st and the 30th September), or earlier in the morning than eight o'clock, or shall leave his usual place of abode without having information where and for what purpose he has gone; or by wilful act or neglect delay his recovery to health; he shall be suspended from receiving any further benefit until the case shall be laid before the Committee, who shall (after enquiry) have power to inflict a penalty not exceeding one week's sick pay, and their decision shall be final. This Rule applies to Sunday as well as to other days. FUNERAL BENEFIT. XIX.— The following allowances will be made in case of death, notice of which must be given to the Secretary, accompanied by a Registrar's Certificate of Death, in every case. In the case of death of husband, wife, or child no claims for Funeral Allowance will be paid except on production of Marriage Certificate. Funeral Benefit will not be paid for any death where the member has been less than 26 weeks in the Society. When subscriptions are 8 weeks in arrears no Funeral Benefit will be paid. All arrears under 8 weeks, together with all fines incurred, must be paid before a member's relative or nominee is entitled to receive Funeral Benefit. Member. Member's Wife.
