If you are claiming Maternity Allowance and your contribution card is in your possession, you should send it with the claim form. If you are claiming Maternity Grant or Attendance Allowance on your husband's insurance you should send your Marriage Certificate. It will be returned to you. 4. How is Maternity Benefit Paid? Maternity Grant is paid either by means of an order sent by post to be cashed at a post office or in cash at your local National Insurance Office. Attendance and Maternity Allowances are paid by means of a book containing Orders which you can cash at a post office chosen by you. MATERNITY GRANT 5. What are the Contribution Conditions for the Grant? Either you or your husband must— 1. have actually paid at least 26 contributions (it does not matter in which class), between the time of entering insurance and the date or expected date of your confinement: and also 2. have paid or been credited (e.g. for weeks of sickness or unemployment) with at least 26 contributions during the period of the last card before the one in use on the date or expected date of your confinement.
