Summary:1948-03 1948 1940s 8 pages If you have not claimed the grant in advance and are not qualified for Maternity Allowance you should arrange to get a claim form from your local National Insurance Office or from the Maternity Clinic and send it with Parts I and IV completed to your local National Insurance Office. The same form can then be used for claiming both the Grant and the Attendance Allowance. The form must be sent in within ten days of your confinement. 9. Are there any rules I must follow? Yes. While you are drawing the allowance you must do no work other than domestic duties in the home and you must take due care of your health. MATERNITY ALLOWANCE 10. What are the Contribution Conditions for Maternity Allowance? 1. For at least 45 out of the 52 weeks up to the date on which the Allowance is due to begin you must have been at work or registered as unemployed or sick, and also 2. In the same 52 weeks you must have paid at least 26 contributions for weeks of work as an employed or self-employed person. But for this purpose a married woman who has chosen not to pay such contributions will be treated as if she had paid a contribution for every week that she worked. If it is necessary to take into account work done before 5th July, 1948, to help fulfil these conditions, only weeks of work insurable under the rules in force at that time can be counted. Maternity Allowance is payable only on the mother's own record. 11. How do I claim Maternity Allowance? On the same claim form as for Maternity Grant (see paragraph 2 of this leaflet) and, if you wish, you can claim both Maternity Grant and Maternity Allowance at the same time.
